1. Driver's door lock actuator failed. 2. Safety Hazard - the controls and key fob no longer lock or unlock the door, which could allow the door to be opened at any time during operation. 3. No, I have not taken it to confirm the door lock is not functioning. 4. No 5. The lock started to show signs of malfunction by not fully locking, then it stopped responding all together.
